Your First Visit:
The first time you come to our office will be a consultation visit. This consists of a patient history overview and physical, and an examination. Dr. Kratz will then share his findings and recommendations for necessary testing or further procedures.
Please Bring With You:
- A list of all prescriptions and over-the-counter medications you currently take
- All insurance cards and information
- A valid driver’s license or photo identification
- Please download and complete the New Patient Packet
NO TESTING WILL BE DONE DURING THE INITIAL VISIT
An emailed report of your visit will immediately be sent to your family physician so that she may share in your treatment.
